Permission to Use Photo and Video

As a photographer taking photos and/or video at the event described below (the “Event”) at the Blair Conference Center, Westfield Inn, or any other property (“Property”) owned by Ohio Farmers Insurance Company and each of its affiliates (collectively, “Westfield”), I hereby grant to Westfield the unrestricted, absolute, perpetual right and license to reproduce, copy, modify, display, distribute, perform, broadcast, transmit, create derivative works from, or otherwise use my photos and/or video taken at the Event for the limited purposes of promoting, marketing, or advertising the Property to the public or within Westfield (the “Works”).

I release and agree to indemnify, defend, and hold harmless Westfield, its agents, employees, licensees, and assignees from any and all claims I or any third party may have now or in the future for invasion of privacy, right of publicity, copyright infringement, or any other cause of action arising out of the use, reproduction, adaptation, distribution, broadcast, performance, or display of my photos and/or videos.

I waive any right to inspect or approve any Works created using my photos and/or videos and waive any claim regarding the final use of my photos and/or videos. I understand the Works may be used at Westfield’s sole discretion, alone or in conjunction with any other material of any kind or nature, except that Westfield will not use the Works for any criminal or illegal purpose or in any manner inconsistent with community standards of decency.

I understand and agree that I am the exclusive owner of all right, title and interest, including copyright, in my photos and/or videos. I hereby grant to Westfield, a worldwide, non-exclusive, fully-paid license to use my photos and/or videos in the Works and all associated intellectual property therein for the purposes articulated above.

I represent and warrant that I am at least 18 years of age and competent to sign this release. 

However, if this release concerns content involving a minor: I certify that I am the parent or legal guardian of the minor referenced in this form, and I provide consent on the minor’s behalf for the collection, use, and reproduction of the minor’s Personal Information for the Works as described above.
