Permission to Use Image and Written/Verbal Statements

As an attendee at Westfield Country Club (“WCC”), I hereby grant to Ohio Farmers Insurance Company and each of its affiliates (collectively, “Westfield”) the unrestricted, absolute, perpetual right and license to reproduce, copy, modify, display, distribute, perform, broadcast, transmit, create derivative works from, and otherwise use my likeness, voice, image, photograph(s), and personal statements, whether written or oral (collectively, the “Personal Information”), in whole or in part, either alone or in combination with other materials, for the limited purposes of marketing WCC to the public through promotional, marketing, advertising, educational, or communication materials (the “Works”).

I release and agree to indemnify, defend, and hold harmless Westfield, its agents, employees, licensees, and assignees from any and all claims I or any third party may have now or in the future for invasion of privacy, right of publicity, copyright infringement, or any other cause of action arising out of the use, reproduction, adaptation, distribution, broadcast, performance, or display of the Personal Information.

I waive any right to inspect or approve any Works created using the Personal Information and waive any claim regarding the final use of the Personal Information. I understand that the Personal Information may be used at Westfield’s sole discretion and that Westfield will not use the Works for any criminal or illegal purpose or in any manner inconsistent with community standards of decency.

I understand and agree that Westfield is and shall be the exclusive owner of all right, title and interest, including copyright, in the Works.

I represent and warrant that I am at least 18 years of age and competent to sign this release. 

However, if this release concerns content involving a minor: I certify that I am the parent or legal guardian of the minor referenced in this form, and I provide consent on the minor’s behalf for the collection, use, and reproduction of the minor’s Personal Information for the Works as described above.
